An analysis of osteosynthesis complications of long bone diaphyseal fractures in patients with polytraumas

Abstract

Background. Polytrauma is characterized by a high level of mortality, and this is largely due to damage to large bones of the skeleton, including the femur. One of the most difficult aspects of the polytrauma problem is to determine the tactics of treatment of diaphyseal fractures of long bones. The aim of the study was to analyze the complications of osteosynthesis of diaphyseal fractures of long bones in patients with polytrauma. Materials and methods. A retrospective analysis of complications of blocked intramedullary osteosynthesis (BIOS) of diaphyseal fractures of long bones was performed in 325 patients with polytrauma. The analysis was carried out differentially depending on the period of complications (early and late postoperative period), as well as depending on the duration of the BIOS (early, early delayed and late). Results. Analysis of the data obtained allowed us to establish that all types of complications, both in the early and late postoperative period, were significantly more often detected during late BIOS. The frequency of complications of BIOS in the groups of early and early delayed intervention was comparable. Conclusions. In most published papers, the incidence of complications of BIOS is lower, but studies were conducted on patients with isolated fractures, not PT. In a few studies of patients with PT, the frequency of major complications of both early and late postoperative periods was comparable with the results we obtained. At the same time, it is emphasized that the severity of the patient's general condition prevents the BIOS from being carried out on the first day after the injury. It seems appropriate to conduct early delayed BIOS of diaphyseal femoral fractures in patients with polytrauma.
